Paper forms for staff
For Sehlam staff. When the care system cannot be used, care carries on: record it on these forms, phone the office, and enter it in the system when it is back.
Download the forms
Each form is a fillable A4 PDF. Save them to your phone or tablet now: they open without a connection, and you can type into them and tick the boxes, or print them and write by hand. The blank forms hold no personal information; once filled in they are confidential.
- All four forms in one PDF (4 pages)
- DT-1 Paper visit record: care staff, one form per visit
- DT-2 Paper medication record: nurses and doctors only
- DT-3 Office call log: the office, for phoned-in arrivals and departures
- DT-4 Reconciliation checklist: the coordinator, when the system is back
If the care system is down
- Go to your visits as planned. If you do not know your next visit, phone the office: they have the day's run sheet.
- Phone the office when you arrive and when you leave. They record the times for you.
- Write each task on a DT-1. Nurses chart medicines on a DT-2, and only from the current, approved order.
- Anything urgent or unsafe: phone the coordinator straight away. In a medical emergency, call the ambulance service first.
- When the system is back, enter the form yourself (on the visit, choose "Enter from a paper form") and hand the paper form to the office at your next visit there.
Keeping the forms safe
Keep a completed form on you during the visit, then hand it to the office for the locked paper-records file. Do not photograph forms or send them by chat. The forms are also kept inside the care system under "If Sehlam is unavailable", and on every phone or computer that has opened it.
